Federated Digital Health Infrastructure for Malaysia
Perdana Architecture · Public design and evidence brief · Version 1.0 · 7 October 2026
Connect systems without replacing institutions
PerdanaHealth’s proposed model connects healthcare services, electronic medical record workflows and workforce capability through a clinician-governed implementation layer. Institutions retain their systems, legal responsibilities and clinical authority; interoperability is commissioned around a defined need rather than assumed across every provider.
HEIS is the implementation methodology. Perdana Architecture defines the operating model. VisionOne is the developing orchestration layer. GPKM is the flagship healthcare-workforce federation in development, with Perdana Academy supporting capability.
Three entry pathways
- Workforce Health & Resilience: A paid 30-Day Health-Economic Architecture Sprint defines one buyer, cohort, priority problem, pathway, owners, KPIs and implementation economics. A separately commissioned 90-Day Demonstrator tests delivery before recurring managed transformation.
- Federated EMR and care coordination: Assess one referral or follow-up pathway, source systems, permissions, interfaces, terminology mapping and delivery cost. A synthetic-data demonstrator precedes any live integration.
- GPKM and Academy: Connect workforce-development requirements to baseline assessment, a personalised development plan, theory and simulation, applied capability, leadership and financial skills, post-assessment and a proposed Perdana Competency & Development Profile.
Public information
The public website provides programme information and research briefs. Institutional integrations and live clinical services are Coming soon, subject to the relevant governance and operational arrangements.
Proposed federation
- Local systems: Existing provider EMRs and institutional workforce systems remain the source systems.
- Interoperability: Document available interfaces, licensed access, identity matching, data provenance and terminology. Use standards such as FHIR where the actual systems and agreed specifications support them; demonstrate conformance rather than describing every connection as FHIR-ready.
- Governance: Define controller and processor responsibilities, lawful purpose, appropriate consent or authorisation, minimum-necessary exchange, role-based access, logging, retention and withdrawal handling.
- Orchestration: Coordinate referrals, acceptance, follow-up, task ownership and escalation within the commissioned pathway.
- Measurement: Establish a baseline for completion, turnaround, rework, adoption, safety and direct operating cost. Publish only reviewed aggregates with disclosure controls.
Federation does not mean data never moves. The exchange design specifies what moves, why, to whom, under which authority and for how long. It is also distinct from federated machine learning, which is not required for this first implementation.
Market and evidence context
Malaysia’s 6 July 2026 RESET statement describes MDHCN as a national trust framework for secure interoperability between public and private healthcare providers under “One Person, One Record” (Ministry of Finance). PerdanaHealth’s strategic response is to develop institution-neutral implementation capability that can be assessed against applicable requirements, rather than duplicate national infrastructure.
A 2024 systematic review of 14 articles identified data availability, quality of care and reuse as reported benefits of semantic interoperability, while noting a small, heterogeneous evidence base and a need for real-world evaluation (Palojoki, Lehtonen and Vuokko, JMIR Medical Informatics). This supports testing semantics, workflow and information quality alongside technical connectivity.
A 2025 thematic review of 161 studies, drawing on PubMed/MEDLINE, Scopus and supplementary sources, identified fragmentation, inconsistent standards and governance challenges; it did not establish PerdanaHealth-specific savings or Malaysian implementation margins (Ambalavanan and colleagues, Frontiers in Health Services). Its design implications are standardised interfaces, semantic consistency, privacy controls and contextual testing.
A 2024 review of 20 educational-intervention studies, searching CINAHL, PubMed, ProQuest, Scopus and Medic, supports multi-method digital-competence education combining knowledge, interaction, practical training and organisational support (Kulju and colleagues, PubMed). This informs Academy design; it is not evidence of Academy accreditation or measured graduate outcomes.
These are peer-reviewed sources retrieved through publisher and PubMed pages, including reviews that searched Scopus. This brief is an evidence-informed design synthesis, not a new systematic review or a direct Scopus database search.
Commercial and cost discipline
The proposed revenue sequence is fixed-scope design, separately priced implementation, recurring managed coordination, and commissioned Academy capability transfer. Technology licensing, clinical delivery, hardware and other pass-through costs are itemised; proprietary development follows repeated customer-funded workflow demand.
The economic evaluation compares the current workflow, configuration of existing tools, and any proposed custom component over the same time horizon. Include delivery labour, founder time, integration, migration, vendor/API charges, hosting, security, training, support, downtime, maintenance and exit costs. Measure savings locally before making a quantified claim.
An implementation should advance only when the buyer accepts the workflow, authorised owners and data boundaries are explicit, tests pass, staff can use it, the support model is funded and the measurement plan is credible. That is the route from a convincing demonstrator to an accountable operating service.
Workforce-first delivery sequence
The initial proposed node is one healthcare profession, one cohort and one accountable buyer. Source institutions retain record authority; purpose-limited exchange, role permissions, auditability and independent security review precede operational use. GPKM is a proposed voluntary professional and welfare community, with PerdanaFlow supporting governed workforce coordination and Academy supporting development.
